We have updated the Forms. This will allow you to save a copy of the form on your local machine so that you can complete and share the form with the members of your team.
When you save a copy on your local machine, we suggest that you leave the name of the form as is. Share the form with your team by attaching it to a new e-mail that you create. Only use the submit button on the form when you are ready to send us your completed form.
We also recommend that you use the latest version Acrobat Reader 8. This is a free program you download from www.adobe.com. If you own previous versions of Adobe Professional or Adobe Acrobat Reader, you might need to uninstall them before you install the new Acrobat Reader or the latest full version of Acrobat Professional 8. The new version has features that might not be compatible with previous versions.
Thank you for your input as we transition. We want to make the reporting process easy for you. Adobe Acrobat 8 Forms allow us to keep the integrity of the forms and the valuable information that you provide to us.

File > Send > Page by E-mail.
> This will prompt the e-mail program.
> Your e-mail program will give you the path where the form is located on your computer.
